3/8/2023 EnchantaVaughn Approved AM) Client was seen grabbing breakfast. Client appeared to be in a restless mood, as evidenced by his laid back demeanor and quiet tone. Client showed excellent participation during his the first therapeutic class activity(e.g. creating music sounds and singing along as a group). Client interacted well with his peers. Client is observed displaying cooperative and compliant behaviors, listening and following directions the first time they are given. (PM) Client was sent out of class for talking over his teacher and being disrespectful. Client seemed to show a lot of undesirable actions due to wanting attention and to get out of doing his work. Client was redirected back to class and requested to pull himself together and not to ruin his day over something so small. Client was able to regroup and follow directions. Client was seen in lunch displaying great social skills. Client expressed positive feelings toward peers and adults. (EOD) Upon returning to class, client became frustrated and was sent out of class twice for a short break. Client was directed to sit up and use his words. Client expressed that he wanted feeling exhausted and tired of doing classwork. Client was encouraged to make better choices and to take advantage of his learning opportunities. Client was acceptive to redirection and returned to class in a cooperative manner. Client showed a high level of participation in group setting and completed all of his work. (AM) QMHP reviewed previous days activities. QMHP inquired about evening and morning and how client was feeling. QMHP asked if he needed help with anything. QMHP encouraged client to have a good day. (PM) QMHP monitored for acquisition of objectives. QMHP encouraged client to stay in class. QMHP encouraged client to be the example. QMHP encouraged client not to follow others that he see are doing wrong. QMHP intervened to assist in explaining behaviors to client. (EOD) QMHP reflected with client on their day, the good and the bad experiences. QMHP what would you do different tomorrow. QMHP encouraged client to have a good evening. QMHP encouraged client that tomorrow is a new day. QMHP praised client for his commitment. QMHP processed client positive responses. QMHP assisted client with recognizing his own personal strengths and how to apply them to reach his goals and objectives. (AM) Client talked about the fun things he learned yesterday. Client said he woke up feeling refreshed and ready to have a good day. Client smiled and hugged QMHP. Client said "No, thank you". Client communicated by nodding and waving. (PM) Client was displaying respectful, non aggressive behaviors. Objectives were being met at this time, considering stressful environment. Client avoided distractions in group setting and was seen paying attention in class. Client exhibited classroom management skills by staying in his seat and contributing to discussion. Client seem to have a big influence on peers and does not follow behind those who he see are doing wrong. Client acknowledged interventions and verbalized understanding. (EOD) Client engaged in reflective thinking. Client communicated the do's and don'ts. Client was in a positive mood. Client said "thank you". Client enjoys being praised, as evidenced by his smile. Client was able to identify all the good things that happened and see the role he plays in each situation. Client applies himself when utilizing his strengths and focusing on his goals and objectives.
